GPU Problems?

So, over the past few months, my GPU performance has gone rather downhill, even to the point where I can't for some reason run at the correct resolution of my monitor.
It's a XFX GTS 250 1 GB, and when I got it it ran like a dream. It's only about 10 months old too. Now, it either has to run games at lower settings, or there are artifacts galore ingame (for example, I can't play Oblivion for 5 min before it starts to artifact a lot.)
So, is this a problem with the GPU itself, or might it be all those new updates Nvidia has been coming out with? I wasn't until they came out with the drivers that also covered the 400 series that it started going really bad.

Hmm, it could very likely be a failed driver Nvidia released, but could also be the GPU that's gotten some kind of damage..
My tip would be to first find and download the older driver you know worked, but don't install yet.. first you uninstall the current driver you have, then run a software called driver sweeper to fully get rid of the driver, then install the driver you downloaded.. if you still have the same issues, send it in to XFX..

Yeah, I'm almost certain it's got to be a driver problem. I popped in the card in another machine that has older drivers, reinstalled them, and it worked fine.
Though, I just did a roll-back on my machine, and it still isn't working DX
The main problem is that my main monitor runs at 1800/1014, but I can only get it to do 1786/1014, no matter how many times I try to resize it. If I try custom resolutions, the digital screen goes larger then the physical one, and almost has a look like the refresh rate is off, even though it's set properly.
