Consider the following scenario:

  A robotic arm is designed by a company, the arm malfunctions and kills the person.

  The person (Person A) who was responsible for the code that caused the arm to malfunction stole the code from another company.
  The person (Person B) who was responsible for testing the code faked the tests after the CEO of the company told her too
  The manager of the team told their team to cut corners after the CEO threatened to fire the team if the project wasn't finished in a timely manner

Depends on the situation and how the arm malfunctioned.
If the malfunction was/is a single case then it's no one fault since it is a mistake in production, maybe the company itself could be responsible but it is no one his fault then.
If the malfunction is a possible threat for everyone and can be redirected to the coding then it is the fault of person A since he coded it, but if he also stole the said code he can also be charged for company espionage.

Long story short: Person A is in deep trouble.

Either when I was trying to decide if I was supposed to enlist and which branch, or when my safeish enlistment plan was shattered and I had to choose between stay civilian or prepare to enlist openly and prepare to be kicked out.
